Who Can Apply and How

Applicants may apply themselves, through family or be referred from institutions or other organizations.

Applicants to the Attendant Outreach Service must be living, or intend to live, in the counties where support services are offered by KPP to access this service.

How to apply:

To obtain application forms call the Main Office - (705) 745-4122 Monday to Friday, 8:30 a.m. to 4:30 p.m. These forms can be mailed out to you or picked up. Once all of the forms are filled out and returned in whole, including the medical component which must be completed by one's doctor, an interview will be arranged to determine if KPP can meet your needs.
